Exploring self-funded healthcare

Self-insured health plans cover over 60% of American workers with employer-sponsored insurance. At the center of nearly every one of those plans is a plan administrator (TPA, ASO, etc.). These organizations handle the day-to-day complexity of benefits administration: adjudicating claims, managing networks, paying providers,, and increasingly, navigating a rapidly evolving vendor landscape.

Around the TPAs exists an entire ecosystem of partners, each solving a specific piece of the puzzle. As we have spent more time exploring this space and the innovation happening in it, we thought we would share what we’ve learned. Today, Amera is publishing our Q1 2026 health plan administration trends and ecosystem map. These are our observations of the exciting changes happening in our space and the organizations making that happen.

Innovations in health plan administration

A few themes have become clear to us over the past year as we have worked on innovating in self-funded healthcare. 

1. AI has made an impact across the TPA landscape, personalizing experiences and helping organizations process messy data; some examples include providing conversational benefit guidance for employees and evaluating proposed care for automating prior auth.

2. Price transparency is becoming more accessible as regulatory requirements (e.g., TiC 2.0) strengthen and more companies are trying to make use of the available data, opening up new opportunities for guidance and savings. Many organizations are exploring options such as reference-based pricing and real-time adjudication / steering.

3. Putting together a compelling and cost-effective plan with so many partners requires a lot of effort, which many employers may not have the bandwidth to do. Success, particularly for innovative plan design, requires making the ROI easy to understand and risk mitigation clear.

This map reflects months of research, conversations with TPA operators and their vendor partners, and our own day-to-day experience sitting at the data intake layer of this industry. We've organized the ~250 companies into four broad groupings:

• Medical Management: the employee-facing vendors that help them make decisions about what benefits to enroll in as well as navigate the complex care landscape

• Cost Containment: the group of companies helping TPAs get the best deal on quality care across a wide range of providers, including RBP vendors, payment integrity firms, utilization management companies, transparent PBM platforms, and other specialty containment solutions

• Networks: the organizations that help TPAs build comprehensive relationships with the providers that care for members

• Enterprise Technology: the less visible but essential layer that ensures data can flow, including claims processors, price transparency tools, data analytics platforms, EDI gateways, provider networks, and credentialing solutions

Where Amera fits

Amera builds claims ingestion and automation infrastructure for TPAs. We help TPAs receive data from providers as well as other partners on this map and process it more accurately and efficiently than legacy manual workflows allow. We also help TPAs automate the process of sharing data back out when needed, reducing manual reporting work (e.g., for stop loss).

We believe that in order to provide the best possible outcomes for members, TPAs require good data infrastructure. If claims data arrives incomplete, out of format, or requiring manual review at every step, decisions become unclear and performance suffers.
